High Cortisol: 7 Signs and Proven Ways to Lower It

Chronic high cortisol raises cardiovascular risk and shows up as abdominal fat, high blood pressure, fatigue and anxiety. Sleep, meditation and moderate exercise are the best documented ways to bring it back down.

High cortisol that stays elevated for weeks leaves recognizable traces: waking at 4 a.m. for no clear reason, a thickening waistline despite a stable diet, tiredness that persists after eight hours of sleep. This adrenal hormone, controlled by the hypothalamic-pituitary-adrenal (HPA) axis, normally rises in the morning to start the day and falls again by evening. When that rhythm breaks down over weeks or months, driven by chronic stress, poor sleep or excessive training, the consequences go well beyond a simple feeling of nervous tension.

In brief – Chronic high cortisol is an imbalance of the hypothalamic-pituitary-adrenal axis that keeps the body in a prolonged state of alert instead of a morning peak followed by an evening decline. A Dutch cohort measured a fivefold higher cardiovascular mortality risk in people in the highest tertile of urinary cortisol (Vogelzangs 2010, n=861). The most common signs are abdominal fat gain, high blood pressure, persistent fatigue despite adequate sleep, anxiety, digestive trouble, weaker immunity and repeated nighttime awakenings. A meta-analysis of 58 trials (Rogerson 2024, n=3508) found that stress-management interventions, meditation and relaxation in particular, significantly reduce cortisol. High cortisol: definition and the role of the HPA axis

Cortisol is produced by the adrenal glands under the control of the hypothalamic-pituitary-adrenal axis: the hypothalamus releases CRH, the pituitary gland responds with ACTH, which in turn stimulates adrenal cortisol production. This circuit normally follows a precise circadian rhythm, peaking about 30 minutes after waking and reaching its lowest point late in the evening. High cortisol becomes a problem when this rhythm flattens out or stays elevated all the time, a state endocrinologists distinguish from Cushing’s syndrome (a rare, pathological excess linked to a tumor or corticosteroid treatment). Mechanism: how chronic stress disrupts cortisol

Repeated exposure to a stressor, whether psychological, physical or linked to sleep loss, keeps the HPA axis continuously activated instead of letting it return to baseline. Several mechanisms combine:

  • Loss of the normal circadian rhythm: cortisol stays high in the evening instead of dropping, which delays falling asleep.
  • Progressive resistance of glucocorticoid receptors, which reduces the body’s ability to brake its own stress response.
  • Interaction with insulin: a cortisol infusion raises insulin levels more sharply in women with abdominal obesity (11.4 plus or minus 2.0 versus 8.2 plus or minus 1.3 mU/l, p less than 0.05), a sign of insulin resistance driven by excess cortisol (Darmon et al. 2006).
  • Redistribution of fat toward the abdominal region, through the action of cortisol on visceral fat cells.

Sleep plays a two-way role in this loop: sleep quality, more than sleep duration, shapes how strongly cortisol reacts to acute stress, according to a study of 73 young adults using the Trier Social Stress Test (Bassett et al. 2015).

What the science says about high cortisol

Cohort data link chronically high cortisol to a markedly higher cardiovascular risk. In a Dutch study that followed older adults for an average of 5.7 years, those in the highest tertile of urinary cortisol had a fivefold higher risk of cardiovascular death (HR 5.00, 95% CI 2.02 to 12.37), among 183 recorded deaths, 41 of them cardiovascular (Vogelzangs et al. 2010). An association does not prove direct causation, but consistency with other markers strengthens the signal.

Hair cortisol, which reflects cumulative exposure over several months rather than a single saliva sample, confirms this trend: in a Dutch case-control study, people in the highest quartile of hair cortisol had a 2.7-fold higher cardiovascular risk (OR 2.7, p=0.01) and a 3.2-fold higher risk of type 2 diabetes (OR 3.2, p=0.04) (Manenschijn et al. 2013).

On the intervention side, a meta-analysis of 58 trials covering 3508 participants compared different stress-management interventions on cortisol levels. The overall effect was positive and moderate in size, with meditation and relaxation techniques ranking among the most effective interventions, ahead of talking therapies (Rogerson et al. 2024). Poor sleep also weakens cortisol regulation under stress, in line with broader data linking sleep duration to all-cause mortality (Cappuccio et al. 2010). According to the World Health Organization, unmanaged chronic stress worsens pre-existing health problems and raises the risk of anxiety or depressive disorders (WHO, Stress).

In practice: 7 warning signs

Chronic high cortisol is not just a feeling of being stressed. Seven signs come up most often in the clinical literature on functional hypercortisolism: abdominal fat gain out of proportion to the rest of the body, new or worsening high blood pressure, persistent fatigue despite adequate sleep time, repeated nighttime awakenings between 2 and 4 a.m., unusual irritability or anxiety, digestive trouble (bloating, irregular bowel habits), and repeated infections reflecting weaker immunity. No single sign confirms excess cortisol on its own: it is their combination and persistence over several weeks that should prompt a biological test with a doctor, the only person qualified to tell a functional chronic stress state apart from Cushing’s syndrome.

Protocol: the best-documented ways to lower cortisol

None of these levers act alone like a switch: they reduce the frequency and intensity of cortisol spikes over several weeks, not in a single day.

  • Set a stable bedtime and wake time, including on weekends, to stabilize the circadian rhythm of cortisol.
  • Practice a daily session of heart rate variability breathing or slow breathing (6 breaths per minute for 5 minutes), a simple format to fit in before a stressful meeting or at bedtime.
  • Introduce a meditation or mindfulness practice for 10 to 15 minutes a day, the intervention most strongly linked to lower cortisol in the Rogerson meta-analysis.
  • Limit caffeine after mid-afternoon, since it can prolong cortisol elevation in sensitive people.
  • Favor moderate physical activity (brisk walking, cycling, swimming) over very intense training repeated without enough recovery, which can instead sustain elevated cortisol.

Frequently asked questions about high cortisol

How do I know if my cortisol is high?

A biological test (saliva cortisol measured at several times of day, hair cortisol or a dexamethasone suppression test) remains the only reliable way to confirm it. Clinical signs (abdominal fat gain, high blood pressure, fatigue, sleep trouble) guide a doctor but are not enough on their own for a diagnosis.

Does high cortisol really cause weight gain?

It mainly promotes fat redistribution toward the abdominal area and increases insulin resistance, as shown in a hydrocortisone infusion study in women with obesity. It does not act alone: diet and physical activity remain decisive factors in overall weight gain.

How long does it take to lower high cortisol?

Controlled trials on meditation and relaxation techniques show measurable effects after several weeks of regular practice, not after a single session. Consistency matters more than the intensity of one relaxation exercise.

Does exercise lower or raise cortisol?

Moderate, regular activity is among the anti-stress interventions associated with better cortisol regulation. Very intense training, repeated without enough recovery, can instead prolong its elevation: the balance between effort and rest matters more than total volume.

Is high cortisol always linked to stress?
