A study published in The Aging Male shows the decline stems from a two-part mechanism: the testes gradually produce less testosterone, and the hypothalamic-pituitary axis that controls them also becomes less responsive with age (Mitchell et al., 2015, PMID 26075536). This dual mechanism explains why andropause progresses along a gentle slope rather than a sharp drop: the two levels of the hormonal system partly compensate for each other over the years.

Testosterone circulates in two forms: bound to carrier proteins, or free and therefore active on tissue. It is this free fraction that declines fastest with age, which explains why symptoms can appear even when total testosterone still falls within a range considered “normal” on a standard blood test.

How Declining Testosterone Affects the Body

Testosterone acts on receptors found in many tissues, not just the reproductive organs. Its gradual decline therefore alters several functions at once:

  • Muscle mass and strength: testosterone binds to androgen receptors in muscle nuclei and satellite cells, which are involved in muscle regeneration (Kim et al., 2022, PMID 36294523).

What the Science Says About Andropause

The Massachusetts Male Aging Study followed 1,156 men for 7 to 10 years: total testosterone fell by an average of 1.6% a year, and the bioactive fraction by 2 to 3% a year (Feldman et al., 2002, PMID 11836290). The European EMAS study, conducted on 3,369 men aged 40 to 79 across eight countries, put the prevalence of symptomatic hypogonadism at 2.1% overall, rising to 5.1% after age 70 (Wu et al., 2010, PMID 20554979).

A meta-analysis of 11 randomized trials in men aged 40 and older confirms that testosterone supplementation increases lean mass and strength in men with a confirmed deficiency (Corona et al., 2022, PMID 34781867). But the benefit comes with caveats: the TRAVERSE trial, conducted on 5,246 hypogonadal men at cardiovascular risk, shows that hormone replacement therapy does not increase major cardiac events compared with placebo, while noting a higher incidence of non-fatal arrhythmias, including atrial fibrillation (Lincoff et al., 2023, PMID 37326322). Hormone therapy is therefore not a trivial step: it should be discussed case by case with a doctor, never self-prescribed.

Andropause: Diagnosis and Practical Benchmarks
